This is a section-by-section help guide to aid in completing the Diablo Day Camp Registration Form. Please also read the "lottery procedures" information.

Campers:

bulletPlease use one form per family.  Please print campers full name.  Campers' birth date, grade next fall and Girl Scout level next fall are all used to place the camper in a unit with girls of the same age/grade.  Troop and School information are used to separate campers so that there are not too many in a unit that already know each other.  The objective of camp is to make new friends. 
 
bulletIf your camper would like to attend camp and be in the same unit with a friend of the same age/grade, please indicate the friends name on the "If possible, place in same group as friend" line and put the friends registration form in the same envelope.  If your envelope is drawn in the lottery, both will be accepted into camp and will be placed in the same unit.
 
bulletIf your camper would like to attend camp and be in the same unit with a friend of the same age/grade; but, the friend has already sent in her registration form, please indicate the friends name on the "If possible, place in same group as friend" line.  If both your envelope and your friends envelope are drawn in the lottery and your friend also indicated her desire to be in the same unit as you, then you and your friend will be placed in the same unit.

Elves:

bulletPlease follow the same general guidelines for campers.  At the time this registration form is completed, the Elf must be 13 years old or entering 8th grade in the Fall.  Elves are placed in sessions as needed.  Every effort will be made to accommodate the session choices.  Elves are not required to attend the same session as their camper siblings.  New Elves are placed in units with older experienced Elves.

Family Information:

bulletThis section must be complete or the registration form will be returned.  If at a later date, you might need confidential information e-mailed to you such as maps and locations of camps or trainings; the e-mail address listed on the registration form will be the only one honored.

Adult Volunteer:

bulletPlease be sure to sign the Volunteer Agreement.  Please visit the Volunteer Training section to learn more about training and the required commitment of the Adult Volunteer.  All adults who work with children at a Girl Scout Camp must complete and file with the Girl Scouts of Northern California:

    1.    Volunteer Application,
    2.    3 Reference Inquiries,
    3.    signed Acknowledgement of Girl Scouts of Northern California Policies,
    4.    a copy of your Drivers' License and
    5.    a copy of your auto insurance policy if you will be
            transporting girls to and from a Girl Scout Camp.
 
bulletThese forms must be completed and be on file with the Girl Scouts of Northern California before Diablo Day Camp begins.  These forms are available to download from the Forms Page on this site.  If you are registering to be an Adult Volunteer at Diablo Day Camp, please download and complete these forms now.

Tags:

bulletTags are 3 to 5 year old, potty trained children of adult volunteers.  The Adult Volunteer must work the same session as their Tag.  The Adult Volunteer can not bring a 3 to 5 year old friend for their Tag to camp.  The Tags' legal Parent/Guardian must be an Adult Volunteer working in the same session.

Boys:

bulletBoys are sons of Adult Volunteers currently in Kindergarten through fifth grade.  The Adult Volunteer must work the same session as their son.
